I'd add to the list the lack of a scalable multihoming solution (something 
being hotly debated).  IPv6 assumes that there is a hierarchy in the network
that is top-down.  Although this might have been true back in the early
90s, multihoming is an extremely common practice today and this doesn't
fit in well with the addressing design.  You can multihome today, but you
basically have to become a provider to do so.
